  • (ON CALL 3-4 DAYS AND 1 WEEKEND IN 6 WEEK PERIOD)

    • MUST INCLUDE PERMANENT HOME ADDRESS
    • TRAVELERS MUST BE GREATER THAN 50 MILES FROM FACILITY
