Job Description:
Seeking a Licensed Physical Therapist Assistant to join an
interdisciplinary team providing care for elderly participants in a
community-based PACE program.
Responsibilities include creating and implementing personalized
care plans, evaluating progress, recommending adjustments, and
promoting independence.
Work involves assisting participants with physical and mental
function, and referring them to internal/external providers as
needed.
Requirements: California licensure, graduation from an accredited
program, outpatient clinic experience preferred, CPR/BLS
certification.
