Rate$19.00 - $24.00 per hour Position Summary: Provides excellent,
professional day-to-day hands-on personal care to individuals with
chronic, serious, or life-limiting conditions. CNAs must exhibit
competencies required by law and abide by Four Seasons policies in
providing personal care to patients, as directed by the written
plan of care. Essential Duties & Responsibilities: Practice within
the scope of license, certification or registration, where
applicable, and as required by law and regulations as delegated to
and directed by the registered nurse: Participates in the
coordination of the patient’s hospice care in accordance with
professional standards of practice Collects data for ongoing
interdisciplinary comprehensive assessments Participates in the
development and evaluation of the plan of care by reporting
observations Contributes to patient and family counseling and
education Perform duties in compliance with Medicare Condition of
Participation Practice safety and infection control methods
Provision of hands-on personal care Works to achieve performance
goals, pursue progress toward individual development plan (IDP) and
demonstrate Four Seasons values and competencies Knowledge, Skills
& Abilities: Must be willing and able to care for patients with
infectious disease processes unless approved accommodations
Knowledge of proper body mechanics Excellent organization skills
Excellent interpersonal skills Excellent written and oral
communication skills Ability to work under pressure to meet
deadlines Ability to take initiative and utilize innovative
techniques and ingenuity Ability to interact with various
departments throughout the organization Ability to work flexible
hours, if needed Ability to participate as a team player to
coordinate projects Ability to read, interpret and apply laws,
rules and regulations Ability to plan, prioritize and coordinate
multiple projects Minimum Qualifications: Education: High School
Diploma/GED Experience: No more than a 24 month lapse in providing
clinical care services upon the completion of a State-Accredited
Certified Nursing Assistant course 0 – 1 year of clinical
experience. Licensures & Certifications: (if applicable) Valid NC
CNA certificate Valid driver’s license and proof of automobile
insurance Summary of Physical Requirements: Walking, standing,
sitting, bending, reaching, twisting, crouching/squatting, kneeling
Flexion/Extension of arms/wrist Hand dexterity to include gripping,
pushing/pulling and writing Lifting/carrying of 100 lbs. often
Intent & Function: All descriptions have been reviewed to ensure

STATUS AS AN AT-WILL EMPLOYER. Other Duties: Demonstrating
continued competency by participating in ongoing education and
training offered by the agency and/or outside resources is a
requirement for each employee’s individual education plan. Hospice
Aides will attend a minimum of 16 hours (includes state required 12
hours) of in-service training yearly by selecting acceptable
learning activities by recognized providers of continuing education
Hospice Aides must exhibit competency in the observance of
Universal Precautions by applying principles of infection control
by: Knowing standard precautions Washing hands before and after
patient care Wearing gloves and other PPE as necessary In
communicating any changes in patient condition to the RN case
manager, Hospice Aides must exhibit competency in: Maintaining
chart documentation of care provided in accordance with the written
plan of care. Maintaining confidentiality and privacy of any PHI
per HIPAA requirements Communicating in a professional manner to
internal/external customers Exhibiting competency in using software
systems such as Outlook and current EMR, to record visits or
treatment and to communicate In maintaining general order, safety
and cleanliness of the patient area, Hospice Aides need to be
competent in applying safety principles by: Disposing of biohazard
materials correctly Being acutely aware and knowledgeable of
personal and environmental safety Demonstrating proper transfer and
lift techniques using good body mechanic techniques Cleaning and
storing DMEs and assisting in storing & tracking patient care
products if applicable In providing excellent patient care, Hospice
Aides need to exhibit competency in treatments including:
Administering personal care Assistance in ambulation or exercises
Assisting with ADL Providing post-mortem care In facilitating
excellent patient care, Hospice Aides need to exhibit competency in
using equipment properly by demonstrating: Use of whirlpool tub
Adjusting air mattresses Using mechanical lift/weights, if
applicable Use of glucometer Exhibiting knowledge of professional
boundaries by: Being able to describe roles and relationships
boundaries as a Hospice Aide Describing inappropriate and
appropriate behaviors in any given patient situation Other duties
as assigned by registered nurse, clinical team leader, EH Manager
or Regional Director of Clinical Services at Elizabeth House
