Agape Care Group is seeking a dedicated Registered Nurse (RN) to join our compassionate team, delivering high-quality hospice and palliative care within our healthcare network across the Southeast. In this role, you will have the opportunity to work closely with patients and their families, providing not only medical care but also support and comfort during challenging times. The ideal candidate will possess a deep passion for end-of-life care, with the ability to manage multiple patient cases with empathy and professionalism. Provide direct patient care to individuals receiving hospice and palliative treatments. Assess, plan, implement and evaluate nursing care plans in collaboration with the healthcare team. Educate patients and their families on disease processes, treatment plans, and palliative care options. Administer medications and treatments as prescribed and monitor responses. Maintain accurate and detailed patient records and reports. Advocate for patients' and families' needs and preferences. Guide family members and caregivers in providing bedside care and comfort. Strong clinical skills in hospice and palliative care settings Excellent communication and interpersonal skills Proficient in EMR (Electronic Medical Records) systems Ability to work compassionately with patients and families facing terminal illness Strong time management and organizational skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Registered Nurse (RN) license in the state of practice CPR certification Preferably certified in Hospice and Palliative Nursing (CHPN)
